AVAILABILITY OF FORM:

The registration forms are available at Holy Heart School, Ajitwal, Moga, between 9:00 AM to 4:00 PM only.

• Fill the form neatly and carefully using a black/blue pen only.

• Ensure that all the columns are properly filled.

SUBMISSION OF FORM:

Submit the registration form along with self-attested photocopies of all relevant documents.

Documents Required at the Time of Admission:

For Nursery Class:
✔ Date of Birth Certificate
✔ Residential Proof
✔ Father’s Qualification Proof
✔ Mother’s Qualification Proof
✔ Medical Certificate (including Blood Group)

For Classes Other than Nursery:
✔ Date of Birth Certificate
✔ Transfer Certificate
✔ Residential Proof
✔ Medical Certificate (including Blood Group)

All submitted forms, details, photographs, and documents will be verified. An informal meeting with the Principal will be scheduled.

Admission Process:

✔ Admission confirmation will be provided on a given date, along with an explanation of the fee structure.
✔ Completion of Admission Form, I-Card Form, and other formalities.
✔ Admission is confirmed only after fee payment.
✔ Fees must be paid at the school fee counter.
